St. Johns Public Schools
If your child will be 5 years old on or before December 1, 2012, they may enroll in Kindergarten for the 2012-13 school year. (This is a State of Michigan Law – no exceptions)
To attend St. Johns Public Schools, the State of Michigan requires the following information be on file before your child may start school:
· Child’s certified birth certificate – this will have the raised seal on it and we will make a copy of it AFTER we verify we have seen the original
· Copy of the child’s up to date immunization records –requirements for a child to enter school have changed, please check with your doctor’s office or local health department for updates
· A physical within the last year – if your child was in pre-school and the physical is less than one year old, you may use it
· A vision and hearing test within the last year – testing will be available at kindergarten registration
· Proof of residency verification
and the following forms will need to be filled out entirely to enable the District to get your child enrolled.
· Student Registration– front and back
· Residency Verification and Internet Access Permission
· Transportation Request Form – if transportation is needed

Please call Jennifer Metzmaker at 989.227.4802 to schedule an appointment for registration day. Print, fill out and return the above forms at Kindergarten Registration in your neighborhood school. You will be bringing your child with you this year to kindergarten registration to meet the staff and enjoy a tour.

East Olive – April 23,
Eureka – April 24,
Gateway – April 26,
Riley – May 1,
Oakview – May 3.
You will be notified this summer about kindergarten placement for your child. This notification will include the school they will attend.
