Nation’s largest PCMH program transforming primary care in Michigan
Blue Cross Blue Shield of Michigan has designated Sparrow St. Johns Professional Associates as a 2013 patient-centered medical home practice. The designation period runs from July 1, 2013 through June 30, 2014.
This designation places the practice among a select group of physicians who are improving primary care in Michigan by adopting the PCMH model of care.
“The patient-centered medical home truly is health care centered on the patient,” said Gregory Holzhei, DO. “We take a comprehensive approach to patient care by helping our patients meet their health goals and keeping track of all their specialist visits, test results and prescriptions.”
Blue Cross Blue Shield of Michigan has designated nearly 1,240 primary care practices – with more than 3,600 primary care doctors – as PCMH practices. The program is the largest of its kind in the country and has the potential to affect 2 million Michigan residents.
With the PCMH model, practices create care teams that work with patients to keep them healthy and monitor their care on an ongoing basis.
Practices coordinate patient health care using registries to track their conditions and ensure they receive the care they need. PCMH-designated practices offer patients more access to the care team, coordinate complimentary care (such as nutrition counseling) and help patients better manage conditions like asthma and diabetes.
A recent analysis of Blue Cross claims data shows that PCMH-designated doctors are succeeding in more effectively managing patients’ care to keep them healthy and prevent complications.
Sparrow St. Johns Professional Associates provides family practice and internal medicine primary care services, surgical services, and coordinates access to specialty care services through consulting physicians.
